Provider Submissions Portal

Document submission:

As a Medicaid provider licensed by ODDS you are required to submit certain types of documentation to your case management entity. The document portal  < linked here >  is intended to make this easy. Each submission will be delivered securely to the Services Coordinator you identify and the information you enter in the forms, helps them to identify what the document is.
