If Late: As per the rules, you'll get a notice if your timesheet is rejected. Make sure to send it in again before the next deadline.

PSW Criminal History Checks


 

Background check forms are sent through email using SignNOW 

  • If you don’t have an email, please create one. Do not use a shared email for SignNOW forms
  • Email psw.info@deschutes.org if you need a new background check or to renew one

 

For new applications:  we will contact the companies that process your background checks and wait for them to approve you. This can take up to five weeks.

For renewal requests:  even after you send us all your forms, the State's system will still send messages to your eXPRS EVV and email about 'expiring DD 84-803 credentials'. You can ignore these messages.


PSW Navigation Services


Provider Navigator Services Overview:

  • You can get up to 20 hours of help from provider navigators.
  • They will guide and support you through joining the program and during the first six months.
  • Local offices can refer you, or you can join on your own.
  • Provider navigators will contact you to make a plan based on your needs.

Provider navigators help by:

  • Watching your progress as you join
  • Helping with paperwork, like the PPL form
  • Solving problems during joining
  • Ensuring you do your background check on time
  • Helping you sign up for orientation
  • Offering trainings in resume building and interviews

Provider navigators also help enrolled providers by:

  • Teaching how to use Carina to find work
  • Explaining how to use Electronic Visit Verification (EVV)
  • Ensuring you complete core training in 120 days
  • Tracking credential dates for timely renewal
  • Offering trainings in time management, planning, computer skills, and communication
